Social Worker, BSW, Psychiatry/Behavioral Science (TCHATT)
JOB DESCRIPTION:
The Social Worker BSW intervenes with patients who have complex psychosocial needs, economic co-morbidities, require assistance with eligibility determination for social programs and funding sources, and qualify for community assistance. To promote patient-centered care, the Social Worker BSW provides care that is respectful of and responsive to individual patient preferences, needs, and values, and ensures that patient values guide all decisions. Supports compliance with treatment protocols, and assists with resource linkage as part of a multidisciplinary healthcare team.
Fulfill duties associated with the TCHATT and CPAN projects through the Department of Psychiatry. Duties will include acting as a liaison with TCHATT providers to locate behavioral health resources to facilitate transition of care of patients from TCHATT to community providers, and to triage calls from Primary Care Providers enrolled in the CPAN project. Supports compliance with treatment protocols.
Min Qualifications:
Bachelor's degree in Social Work; no experience required. Must be licensed to practice as a Social Worker in Texas (LBSW).
Specific Job Related Duties:
- Demonstrate competency and skill in communicating with patients, families and school personnel.
- Participate in team meetings and provide status updates to team members and leadership.
- Participate in conferences, workshops, and trainings related to your role in CPAN.
- Assist with maintaining project data and required reports
- Collaborate with program clinicians to cording ongoing care for TCHATT patients
- Disposition TCHATT cases upon discharge from services
- Develop care plans for patients and act as liaison between patients and clinicians
- Assist with development and maintenance of mental health resource database
- Triage calls received by UTMB’s CPAN team.
- Assist with the provision of referrals and resources for mental health services to CPAN providers
Preferred Experience:
- 1 year experience working with families.
- Must have proficient computer skills.
Preferred License/Certificate
Must be licensed as a Social Worker in Texas. (LBSW)
Salary Range:
$50,000 - $55,000
